FINAL FORMS – Remember, if you have any changes to your information (phone number, address, who can pick up your student, etc) throughout the school year, change it here. The office is notified of these changes and will update our information.
COALITION FOR A HEALTHY COMMUNITY Report
Attention 7th Grade Parents:
It is required by Ohio School Law to show proof of the following immunizations BEFORE the start of the 7th grade school year:
Tdap (tetanus, diphtheria, and pertussis) vaccine
MCV4 (meningococcal) vaccine
Deadline for immunizations is October 1st, 2022. If we do not have documentation of the required vaccines by this date then your child will be excluded from school on October 3rd, 2022.
Please check with your medical provider to see if your student has had these immunizations. Immunization records can be faxed to 513-273-3303.
Please call the clinic nurse at 513-273-3314 with any questions or if you need resources on where to get your student vaccinated.
